How to create and edit sign-up forms in Klaviyo Starter in Shopify

Learn how to create, edit, publish, and remove sign-up forms in the Klaviyo app so you can grow your subscriber list directly from your storefront.

Before you begin

Requirements: Forms only display on your storefront when the Klaviyo app embed (Klaviyo Onsite Javascript) is enabled in your Shopify theme. If it's off, your dashboard shows an App embed is not enabled warning with an Open theme editor link. Enable the embed and save your theme first.

Create a form

1. From your dashboard, find Ways I'm collecting subscribers and click to create a new form.
2. Pick a layout. Each option is already styled with your store's branding.
3. Customize the content in the editor.
4. Click Create form to publish it live right away (you'll see a Form created. confirmation), or Save as draft to keep it unpublished (Draft saved.).

Subscribers who sign up through your form join your email marketing list, which also triggers your welcome email if it's live.

Edit a form

1. On the dashboard, open the form from the Ways I'm collecting subscribers table.
2. Make your changes in the editor. Klaviyo re-applies your store's current theme branding, so a form created before a theme refresh picks up your new look when you save.
3. Click Save.

Note: Saving an edit replaces the form with an updated copy that keeps the same name and live/draft status. Because it's a new copy, the form's performance stats (submitted count and submit rate) start fresh after an edit. If you see a message that the previous copy couldn't be removed, delete the older duplicate from the dashboard.

Publish or unpublish a form

Use Set Live or Set Draft in the editor to control whether the form shows on your storefront.

Note: Set Live and Set Draft act on the last saved version of the form. If you have unsaved edits in the editor, save them first, then change the status. Otherwise the storefront shows the form without your latest edits.

Delete a form

1. On the dashboard, open the row menu for the form in Ways I'm collecting subscribers.
2. Click Delete and confirm. You'll see Form deleted.

Deleting a form doesn't remove the subscribers it already collected; they stay on your email marketing list.
